Administrative Forms (required)
Every player must complete the following forms (clicking these links will open the document in a new window)

1. Player and Parent Code of Conduct
2. Concussion Awareness
3. Medical Release

Registration Forms (required)
Additionally, you will need to complete a registration form for the level of competition your player's team is competing
MSPSP (Premier) Registration Form
WSSL (Select and Recreation) Registration Form

Proof of Player's Age
Finally, you may also need to provide a copy of your player's birth certificate.  The copy does not need to be certified.
